Hospital Transition Support
Hospital transition support is a specialised service offered through the National Disability Insurance Scheme (NDIS) that helps participants leave hospital safely and return to their home or community. When you're ready to be discharged from hospital, having the right supports in place can make all the difference to your recovery and wellbeing.
Transitioning from hospital can feel overwhelming.
You may have experienced changes to your health or disability, and navigating what comes next requires careful planning and coordination. Our hospital transition support services are designed to ensure you have the supports you need as you adjust back to life after your stay in the hospital.
Meliora Life provides Hospital Transition Support services throughout South East Queensland (Moreton Bay Regional, Ipswich City, Brisbane City, Logan City and Redland City).

Meliora Life specialises in providing smooth, coordinated transitions from hospital to home or community. Our experienced team works closely with you, your hospital team, and other service providers to ensure your discharge is as safe and stress-free as possible.
We understand that every person's situation is unique. Whether you're recovering from surgery, adjusting to an acquired brain injury, managing a mental health condition, or experiencing changes to your disability support needs, we provide consistent, person-centred support throughout your transition.
At Meliora Life, our trained and experienced Support Coordinators and Support Workers can assist you with:
Discharge planning and coordination: Working with hospital staff, NDIS planners, and other professionals to ensure all necessary supports are arranged before you leave hospital.
Support coordination: Helping you understand your NDIS plan, connecting with quality providers, and ensuring your supports are working well for your transition needs.
Personal care support: Arranging assistance with daily activities like showering, dressing, meals, and medication management as you settle back home.
Home modifications and assistive technology: Coordinating necessary equipment like hospital beds, mobility aids, grab rails, or ramps to support your safety and independence at home.
Allied health services: Connecting you with physiotherapy, occupational therapy, speech therapy, or psychology services to support your ongoing recovery.
Short-term accommodation: If returning home immediately isn't the right option, we can arrange temporary accommodation while you build skills or wait for longer-term housing arrangements.
Community reconnection: Support to maintain connections with family and friends, and assistance to participate in activities that matter to you.
Advocacy and communication: Representing your preferences and needs with health providers, NDIS planners, and other services involved in your care.

Hospital Transition Support is funded in your NDIS plan under the following categories:
Support Coordination (Capacity Building Supports)
Specialist Support Coordination (Capacity Building Supports)
Assistance with Daily Life (Core Supports)
Assistance with Social and Community Participation (Core Supports)
Medium Term Accommodation (Capital Supports – if eligible)
Home Modifications (Capital Supports – if needed)
Assistive Technology (Capital Supports – if needed)
If you're currently in hospital and don't have an NDIS plan, or if your existing plan doesn't include the supports you need for discharge, the NDIS has specialist hospital discharge teams who work quickly to approve plans and supports. Our team can work with NDIS Health Liaison Officers to ensure funding is in place before you leave hospital.
